Strengthening community-based efforts in surveillance, immunization, maternal and child health, and nutrition services to address vaccine-preventable and priority zoonotic diseases, including polio and malaria, among hard-to-reach populations.

The CORE Group Partners Project (CGPP) is a multi-country, multi-partner initiative that works with governments and civil society to strengthen immunization systems, disease surveillance, and essential community-based health services in hard-to-reach and underserved communities. Rooted in global polio eradication, CGPP leverages decades of experience and community networks to improve routine immunization, detect and respond to infectious disease threats, and reach mobile and cross-border populations. Through strong partnerships and data-driven approaches, CGPP helps countries prevent outbreaks and protect health across borders.

FY25 accomplishments

“I feel reassured knowing that my daughter is up to date with her vaccinations. Thanks to the project’s support and the close work of Maman Sani, we feel a real relief in our daily lives.”

Faiza Hachimou, Community Member, Mother, CGPP Niger

“We serve the same community. Whenever I see something unusual that resembles a disease, I inform the community mobilizer, who reports to the health facility. This way, our community stays safe.”

Ali Galgalo, Traditional Healer, CGPP Kenya

“While in South Sudan, I never took any of my children for immunization because I didn’t think it was important...But when I arrived in Elegu, Uganda, I was told about the importance of immunizing all my children. I was convinced by a person who was speaking the same language as mine that the vaccine was good. Because of that, I decided to accept my five children to be vaccinated against measles and polio.”

Majok Deng, community member, CGPP Uganda

“When i see a healthy child smiling after vaccination, all the effort feels worth it.”

Buke Guyo, EPI Focal Person, CGPP Ethiopia

“I used to think health workers only went to the big towns. Now I know they care about us too.”

Deqo Arte, Community Member, CGPP Somalia.

“During the pandemic, CGPP trained me on the symptoms of COVID and preventive measures. Apart from gaining knowledge on disease prevention, I enhanced my communication skills, particularly on how to establish faith in the community, build rapport, and listen to the problems with empathy before responding.”

Rita Rani, communication expert group member, CGPP India

“I have not heard of anyone who got vaccinated in this village. I am happy to be the first, not only in my family but also in my community. If the vaccine was not brought to us, no one would have got it.”

Natabo, community member, CGPP South Sudan
